    Key Responsibilities

    • Conduct comprehensive assessments of clients’ needs and develop individualized support plans.
    • Provide case management services, including advocacy, referrals, and support in accessing resources such as housing, healthcare, and social services.
    • Monitor clients’ progress and adjust support plans as needed.
    • Collaborate with a multidisciplinary team, including healthcare professionals, social workers, and housing providers.
    • Maintain accurate and up-to-date client records.

    Requirements

    • Minimum of one year of experience in social services or a related field.
    • Knowledge of community resources and social service systems in Vancouver.
    • Strong communication, interpersonal, and problem-solving skills.
    • Ability to work independently and as part of a team.
