Key Responsibilities
- Conduct comprehensive assessments of clients’ needs and develop individualized support plans.
- Provide case management services, including advocacy, referrals, and support in accessing resources such as housing, healthcare, and social services.
- Monitor clients’ progress and adjust support plans as needed.
- Collaborate with a multidisciplinary team, including healthcare professionals, social workers, and housing providers.
- Maintain accurate and up-to-date client records.
Requirements
- Minimum of one year of experience in social services or a related field.
- Knowledge of community resources and social service systems in Vancouver.
- Strong communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team.
